Physician-Authored Medical Evaluations and Transparent Pricing
David Medical Services provides independent, physician-authored medical opinions for workplace accommodations, disability benefits, and military matters. Each service addresses a specific medical question, with fees reflecting the complexity of the evaluation, records reviewed, and level of analysis required.
Independent physician opinions for federal and private-sector employees who need physician documentation explaining how a medical condition affects work-related function and supports a requested accommodation.
Best for relatively straightforward cases involving an established medical condition, defined functional limitations, and a specific accommodation request.
Best for more complex cases involving multiple conditions, several job functions, telework, travel or fieldwork restrictions, attendance issues, or prior insufficient documentation.
For federal employees whose medical conditions may prevent continued useful and efficient service despite treatment and appropriate accommodation efforts.
A focused physician assessment of whether the available evidence appears capable of supporting a Federal Disability Retirement medical opinion. If the assessment determines the evidence supports a Federal Disability Retirement medical opinion, the fee for this Level 0 review is applied toward a Level 2 or Level 3 Federal Disability Retirement Independent Medical Opinion service (see below for details).
A physician-authored summary of the relevant medical history, functional limitations, and work-capacity issues.

Independent physician review of the medical evidence supporting military decorations.
Before preparing a formal independent medical opinion, our physician must first determine whether the available evidence provides a reasonable medical foundation for the requested conclusions. This Level 0 review includes physician review of up to 25 pages of relevant evidentiary records, e.g. military, medical, and supporting documentation.
If the Level 0 assessment finds a formal medical opinion supportable, we will provide a scope of work and fee based on the medical questions involved, complexity of the case, and volume of records requiring review. The $500 Level 0 fee is credited toward a subsequent Military Award IMO addressing the same injury and award request.

Independent physician review of how a Veteran's service-connected medical conditions affect their ability to work.
An initial physician review to determine whether the available medical evidence appears sufficient to support a formal occupational functional opinion. Includes review of up to 25 pages of evidentiary records.
A formal, physician-authored medical opinion evaluating the occupational effects of the veteran's service-connected disabilities. Includes review of up to 100 pages of relevant medical records and supporting documentation.
An expanded medical assessment for complex cases requiring additional clarification of functional limitations, symptom patterns, or occupational history. Includes up to 150 pages of medical records and supporting documentation and provides an expanded analysis of occupational functioning.
Fees cover assessment of the service-connected conditions relevant to one TDIU claim. Particularly complex cases may require an individualized fee estimate, which will be provided before work begins.
Each service includes the record volume specified for that evaluation. Because physician review time increases significantly with larger record sets, additional records are billed at
$250 (for additional physician review time) per additional 25-page increment.
Cases that involve substantially larger record sets, unusually complex medical questions, multiple conditions, reconsideration or appeal, prior adverse decisions, or attorney-directed analysis may require additional review fees or individualized pricing. If additional review is needed beyond the agreed scope, we will identify the anticipated fee before proceeding.
Our fees cover independent physician review, medical analysis, and preparation of the agreed service when medically supportable. Payment does not guarantee:
